The most expensive construction risk may be the one that never appears in the incident report.

A change in site conditions may seem minor at the time. Weather shifts. Work sequencing changes. Materials arrive differently than expected. A control is adjusted in the field, but no one records what changed, when, or why.

Later, safety, compliance, cost, and responsibility questions surface. Reconstructing the sequence from memory is not the conclusion. It is uncertainty.

A simple changed-conditions log can preserve the evidence:

• Time and location
• Condition observed
• Responsible party
• Control or procedure updated
• Supporting photos
