🧐 🧐 🧐 Langerhans cell histiocytosis ( ) is a rare condition characterized by abnormal growth of the Langerhans cells. The proliferation of these cells leads to the formation of tumors called granulomas, which can then invade the bones and cause pain, inflammation and (in some instances) bone fracture. The disease is most commonly seen in children younger than 15 years of age; however, it has been observed in other age groups too.

📌 📌 📌 The exact etiology of LCH is unknown; however, somatic mutations in the gene have been identified in the majority of the patients. In a nutshell, BRAF gene encodes a protein called B-Raf, which is involved in transmitting growth signals to the nucleus of the cell. Consequently, somatic mutations in the BRAF gene lead to increased production of the B-Raf protein, which ultimately causes uncontrollable cell proliferation.

🔬 🔬 🔬 The histological features of LCH are variable and influenced by the age of lesions and their location. Generally speaking, on microscopy, cells are oval or round with a nuclear groove resembling a coffee bean. LCH coalesces as an eosinophilic granuloma, owing to the influx of eosinophils, macrophages, T cells, and multinucleated cells. Immunostaining highlights the expression of CD1a, Langerin (CD207), and S100.

📢 📢 📢 (CD207) is a type II transmembrane protein which is encoded by the CD207 gene in humans. It is a main part of Birbeck granules. Langerin is C-type lectin receptor on Langerhans cells (LCs). It is the most specific marker for Langerhans cells and is functionally involved in antigen uptake.

📣 📣 📣 (Cluster of Differentiation 1a) is a human protein encoded by the CD1A gene. CD1a is a β2 microglobulin associated transmembrane glycoprotein of the CD1 family that presents lipid and glycolipid antigens to T cells and serves as a diagnostic marker for Langerhans cells and selected immature T cell neoplasms.

📌 Anaplastic lymphoma kinase (ALK) is an enzyme that in humans is encoded by the ALK gene. Normal ALK is expressed only in embryonic nerves and plays a role in nervous system development. As adults, ALK is mostly silent and rarely express themselves.

💥 However, in tumors, due to ALK gene rearrangements, point mutations, and amplification, ALK kinase activity is permanently and uncontrolled, driving tumor development.

‼️ ALK-positive anaplastic large cell lymphoma (ALK+ ALCL) is a type of T-cell lymphoma, most commonly seen in children and young adults. Due to a t(2;5) chromosome translocation, an NPM-ALK gene fusion occurs, resulting in persistent ALK activation and driving lymphoma development.

‼️ ALK fusion-positive non-small cell lung cancer (NSCLC) occurs in 3%–5% of NSCLC patients. Due to an inversion of an internal segment of chromosome 2, an EML4-ALK gene fusion occurs, forming a constitutively activated kinase that strongly drives the tumor.

❓ What is Low-grade osteosarcomas 🤔
🔔 is a malignant bone-forming tumor that produces osteoid or immature bone, but behaves far less aggressively than its high-grade counterpart. Although slow-growing, it remains a malignant tumor requiring surgery.
🔔 Under a microscope, these cells look more like normal bone cells and have few dividing cells. These tumors are more likely to form on the outermost layer of the bone covering, called the periosteum, or on the surface of the bone (periosteal). In rare cases they can occur in the center of the bone (low-grade central osteosarcoma).

📢 Low-Grade Osteosarcomas vs. Benign Bone Lesions 😶
🔔 These two categories can be notoriously difficult to distinguish — both radiographically and histologically — which is precisely why IHC and careful pathological analysis matters.

‼️ MDM2 / CDK4 Amplification (the key molecular differentiator)
✅ Low-grade central osteosarcoma and parosteal osteosarcoma characteristically show MDM2 and CDK4 gene amplification — detectable by FISH or IHC.
✅ Benign bone lesions are negative for MDM2/CDK4 amplification.
✅ This is arguably the single most important distinguishing marker in difficult cases.

💥 MDM2 & CDK4 IHC Expression in Low-Grade Osteosarcoma
🔔 Low-grade osteosarcoma shows nuclear positivity for MDM2 and CDK4 antibodies, with moderate to strong staining intensity. The positive staining can be seen in majority (around 80-95%) of low-grade osteosarcomas. If a low-grade osteosarcoma dedifferentiates into high-grade, MDM2/CDK4 amplification is retained in the high-grade component — so these markers remain useful even in transformed tumors.

📌 h-Caldesmon and Cancer
✨ In most cancers (e.g., breast, colorectal, and liver cancer), the expression of h‑Caldesmon is downregulated or its activity is lost, resulting in a weakened inhibitory effect on cell migration. It removes the constraints on cell movement, thereby significantly enhancing the invasion and metastatic potential of tumor cells.
✨ In certain tumors (e.g., pancreatic cancer and mesothelioma), h‑Caldesmon expression is conversely upregulated and it is associated with tumor angiogenesis, drug resistance, and poor prognosis.

📌 Application of h‑Caldesmon for Pathological Diagnosis
✨ h-Caldesmon is a marker for the identification of smooth muscle. Diffuse and strong positivity for h‑Caldesmon is one of the evidences supporting the diagnosis of leiomyoma or leiomyosarcoma. In the differential diagnosis of leiomyosarcoma versus myofibroblastic proliferative or neoplastic lesions such as myofibroblastic sarcoma and nodular fasciitis, positivity for h‑Caldesmon is indicative of leiomyosarcoma.

📌 The combined use of h‑Caldesmon and CD10 helps to differentiate uterine mesenchymal tumors
✨ Uterine smooth muscle tumors (leiomyoma / leiomyosarcoma) typically show strong positivity for h‑Caldesmon, but they are negative or only focally weakly positive for CD10.
✨ Endometrial stromal sarcomas characteristically exhibit diffuse positivity for CD10, but they are negative for h‑Caldesmon.

✅ BRAF is a human gene that encodes a protein called B-Raf. The gene is also referred to as proto-oncogene B-Raf and v-Raf murine sarcoma viral oncogene homolog B, while the protein is more formally known as serine/threonine-protein kinase B-Raf.

❓ What is BRAF-V600E ❓
✅ More than 30 mutations of the BRAF gene associated with human cancers have been identified. In 90% of the cases, thymine is substituted with adenine at nucleotide 1799. This leads to valine (V) being substituted for by glutamate (E) at codon 600 (now referred to as V600E) in the activation segment that has been found in human cancers.

📢 The carcinogenic mechanism of BRAF-V600E 📌
✅ The V600E mutation causes constitutive activation of BRAF protein kinase activity, enabling sustained activation of downstream signaling pathways independently of upstream stimuli, ultimately leading to aberrant cell proliferation, resistance to apoptosis, and tumor progression.

✅ This mutation has been widely observed in papillary thyroid carcinoma, colorectal cancer, melanoma and non-small-cell lung cancer.
🔷 High-prevalence tumors: Papillary thyroid carcinoma (approximately 45%), cutaneous melanoma (40%–60%), and low-grade/serous borderline ovarian carcinoma (approximately 35%).
🔷 Moderate-prevalence tumors: Colorectal cancer (5%–15%).
🔷 Low-prevalence tumors: Non-small cell lung cancer (1%–3%).
🔹 This mutation is also detected in some benign precursor lesions (e.g., colonic serrated adenomas), suggesting that it may represent an early event in tumorigenesis and drive malignant transformation.

📢 Detection of BRAF V600E mutation by immunohistochemistry (IHC)
✅ IHC is a widely used clinical detection method for BRAF V600E mutation. In positive tumor tissue sections, tumor cells typically show diffuse and uniform cytoplasmic staining with the specific BRAF V600E antibody; the staining intensity is generally consistent, and non-tumor cells (such as stromal cells or inflammatory cells) usually do not exhibit specific staining, serving as internal negative controls.
