🌍 When Cholera Reveals a Hidden Threat to the Nervous System - Case report

During a cholera outbreak in rural Yemen, a 12-year-old boy developed rapidly progressing paralysis far more alarming than the infection itself. What initially seemed like a routine post-infectious complication turned out to be Guillain–Barré Syndrome (GBS).

Diagnosing GBS in the middle of an epidemic in a prolonged conflict and fragile healthcare infrastructure is never easy. However, thanks to international laboratory collaboration and advanced immunological testing, our team was able to support in uncovering the cause. Using line immunoassays automated by DotDiver2.0, we identified antibodies against specific gangliosides markers of the immune system mistakenly targeting the nervous system. In addition to other clinical and diagnostic assessments, this confirmed a diagnosis of GBS.

Early intervention with IVIG therapy and close rehabilitation proved lifesaving. Beyond the individual case, this story shows how infectious outbreaks can unexpectedly trigger autoimmune complications, and how cutting-edge diagnostic tools can make all the difference -even in resource-limited settings.

Anti-GAD65 Antibodies: Why Assay Performance Matters at High Titers

Anti-GAD65 antibodies are well-established biomarkers in autoimmune diagnostics. Low to moderate titers support the diagnosis of type 1 diabetes, while very high titers are strongly associated with neurological autoimmune disorders such as stiff-person syndrome, autoimmune encephalitis, cerebellar ataxia, and epilepsy.

A recently published study (Verdú et al., 2025) compared a commercial CLIA with the Medipan anti-GAD65 RIA in patients with autoimmune diabetes and neurological GAD-spectrum disorders. The results highlight critical analytical challenges at extreme antibody concentrations including proportional bias, non-linearity, and the risk of silent underestimation due to hook effect.

These findings underline why understanding assay behavior at high titers is essential for accurate clinical interpretation and patient stratification.
