SCENARIO ROOM: Two alarms go off at the same time. Which one do you respond to first?πŸ€”πŸ˜

Imagine you’re on monitoring duty in a gas processing plant when two alarms appear almost simultaneously:

πŸ“A high-high discharge pressure on a compressor& πŸ“A low level on a separator vessel. Both are legitimate process alarms. Both could develop into a bigger problem if ignored, and both went off at the same time.

So where does your attention go first?
The obvious answer might be to pick the alarm with the highest priority or the one that looks most dangerous. But that’s where things get interesting.

Alarm response isn’t supposed to be a race to silence the loudest notification. The operator has to consider the process state, alarm priority, rate of change, potential consequences, and available protective layers before deciding what demands immediate action.

A compressor pressure alarm might indicate an escalating condition that requires intervention within seconds. But if the separator level is falling rapidly because of a developing upstream problem, ignoring it could create a different chain of events.

The point is this:

The alarm itself doesn't tell you the whole story. The process does.
So, if you were sitting in that control room, which alarm would you investigate first, and what information would you check before making that decision?

PSV vs PRV β€” two valves, one very important job: managing overpressure.

But are they actually the same?

Not exactly.

A Pressure Safety Valve (PSV) is generally designed for rapid pressure relief, particularly in gas or vapour service, while a Pressure Relief Valve (PRV) typically opens progressively to relieve excess pressure.

The catch? The terminology can vary across industries, manufacturers and standards. So simply seeing β€œPSV” or β€œPRV” on a valve isn’t enough to understand its function.

The real engineering question is: What is the valve designed to do, where is it being used, and what standard governs it?

Because when you’re dealing with pressure, the difference between a label and actual valve behaviour matters.

Would you agree that the most dangerous phrase in engineering is:
"We've always done it this way."

Not because experience is a bad thing. Quite the opposite. Experience is invaluable. But when it stops us from questioning assumptions, reviewing old practices, or embracing better solutions, it can quietly become a liability.

Many equipment failures don't begin with a catastrophic event. They begin with small compromises that become routine, procedures that are no longer challenged, and workarounds that slowly become the new standard.

The safest and most reliable plants aren't necessarily the ones with the newest technology. They're the ones where engineers never stop asking, "Is this still the best way?"

What's one engineering practice you've seen that people continue simply because "it's always been done that way"?
