In leptomeningeal disease, MRI and cytology are often inconclusive. Getting to a clear answer matters.

A new peer-reviewed study in Diagnostics evaluated VAF from CSF using Belay's Summit™ assay across 118 specimens, with findings that suggest VAF may provide a quantitative signal to support both LMD diagnosis and treatment monitoring.

▸ When imaging and cytology were inconclusive, elevated VAF prompted further workup that led to confirmed LMD diagnoses.

▸ A VAF of 5% or higher was found in 69% of confirmed LMD cases, while most non-LMD cases fell below that level, suggesting VAF may help differentiate LMD from other CNS presentations.

▸ In patients undergoing treatment, repeat CSF testing showed VAF tracked with clinical course: falling VAF correlated with improvement, and rising VAF corresponded with disease progression.

▸ In three cases, a new variant on repeat testing suggested clonal evolution or spatial heterogeneity within the leptomeningeal space.

The analytical and clinical validation of Summit™ 2.0, our CSF-based comprehensive genomic profiling test for CNS malignancies, has been published in Cancers, reinforcing the impressive performance of our flagship test compared to tissue, the current gold standard.

In the 118 sample clinical validation cohort, Summit™ 2.0 demonstrated:
• 96% clinical sensitivity and 98% clinical specificity
• 100% detection rate with a limit of detection of 0.3% VAF
• Comprehensive genomic profiling across 500+ genes

As Alexandra Miller, MD, neuro-oncologist at NYU Perlmutter Cancer Center, shared in a recent Genome Web article:

“This type of assay is excellent for diagnosis in patients where tissue biopsy is not feasible, unsafe, or non-diagnostic, and for cases where additional molecular information could meaningfully guide therapy or help identify patients for clinical trial enrollment.”

Big News! 🎉 Belay Diagnostics is proud to announce the peer-reviewed publication of the analytical and clinical validation of Summit™ 2.0 Comprehensive Genomic Profile, our best-in-class cerebrospinal fluid (CSF)-based liquid biopsy for primary and secondary central nervous system (CNS) tumors. In a new study published in Cancers, Summit 2.0 was clinically validated across 118 clinical CSF samples, which were either tissue-matched or had a definitive diagnosis. Summit™ 2.0 is now the only CSF liquid biopsy available that is clinically validated against the gold standard of tumor tissue for CNS malignancies.

For many CNS cancer patients, tumor tissue is inaccessible, and plasma testing routinely fails due to the blood–brain barrier. Summit™ 2.0 analyzes tumor nucleic acid directly from CSF to detect SNVs, MNVs, Indels, CNVs, fusions, TMB, and MSI, all from just 6 mL of CSF.

Belay Diagnostics is announcing a newly published case highlighting how Summit™, our CSF-based liquid biopsy, was used to monitor leptomeningeal disease (LMD) and guide therapy adjustments in a patient with metastatic lung cancer.

By analyzing CSF rather than plasma, clinicians uncovered key genetic differences between CNS and systemic disease, revealing insights standard plasma testing couldn’t provide. Summit’s high sensitivity enabled detection of ultra-low variant allele frequencies (VAFs), allowing the treating physician to track treatment response and safely extend therapy intervals, minimizing toxicity while maintaining disease control.

This case illustrates how Summit™ can do more than assist in confirming diagnosis; it's a dynamic tool for evaluating treatment efficacy, uncovering genetic divergence, and supporting personalized decision making in complex neuro oncology care.

We had an amazing experience at the annual Society of Neuro Oncology conference last week. Belay Diagnostics was a proud exhibitor again at this year’s meeting.
