The hardest problem has absolutely shifted from materials to regulatory. Here is why:

The materials science for medical AM is largely solved. Titanium Ti-6Al-4V ELI, PEEK, hydroxyapatite, bioresorbable polymers — these all have decades of clinical evidence. The engineering exists. The printers exist. The post-processing exists.

What does not exist is a regulatory framework that was designed FOR additive manufacturing. The FDA, EMA, and most national regulators still evaluate 3D printed implants using frameworks built for conventionally manufactured devices. This creates three specific bottlenecks:

1. The 510(k) problem — most medical device clearance relies on "substantial equivalence" to a predicate device. But a patient-specific 3D printed cranial implant has no predicate by definition. Every case is unique. The system was not built for this.
2. The point-of-care problem — when a hospital prints its own implant, who holds the manufacturing license? The hospital? The surgeon? The software vendor? The printer manufacturer? Nobody has a clean answer yet.
3. The procurement problem — even with regulatory clearance, hospital procurement departments require validated suppliers, ISO certifications, and liability frameworks that most AM companies — especially smaller ones — cannot yet provide at scale.

The company that wins will have solved:

A repeatable regulatory submission pathway
A liability framework for point-of-care printing
Hospital procurement integration

Not better materials. Better paperwork.
