Biosplice is developing first-in-class, small-molecule therapeutics based on pioneering science of alternative pre-mRNA splicing. Stemming from foundational discoveries in Wnt pathway modulation, Biosplice has elucidated novel biology linking CLK/DYRK kinases to the therapeutic regulation of alternative splicing. Alternative splicing is an essential biological mechanism that regulates the diversification of proteins in a cell, which, in turn, determines cell type and function. Biosplice’s target class governs the selection of tissue-specific mRNA splice sites, making them attractive, druggable targets within the cellular “command and control” center. Biosplice’s drugs in clinical development include lorecivivint for osteoarthritis (in Phase 3), cirtuvivint for numerous cancers, and a broad pipeline that ranges from Alzheimer’s disease to other degenerative conditions. In an 18-month community trial in North Carolina, 823 adults aged 50 and over with knee OA and overweight or obesity were assigned to a diet-and-exercise program or an attention-control group. The program group lost an average of 8% of body weight, versus 2%. Knee pain was lower in the program group — a statistically significant but small difference that the researchers called of uncertain clinical importance.

There's no cure for knee osteoarthritis, so every current international guideline recommends exercise to self-manage symptoms. A 2024 Cochrane review of 139 trials (12,468 people) found exercise probably improves pain, physical function and quality of life in the short term — but by amounts of uncertain clinical importance, based on low- to moderate-certainty evidence.

In a Finnish trial, 146 adults aged 35 to 65 with a degenerative meniscus tear and no clinical signs of knee OA were randomly assigned to arthroscopic partial meniscectomy or placebo surgery — a diagnostic arthroscopy with nothing removed. Five years later there were no relevant differences between the groups in knee symptoms or function, and the researchers reported a slightly greater risk of X-ray knee OA progression in the surgery group.

In a US study of 1,788 adults with or at risk of knee OA who wore step counters for a week, each additional 1,000 steps a day was associated with a 16–18% lower risk of developing functional limitation over the next two years. About 6,000 steps a day best separated who did and didn't develop it — a preliminary estimate. It's observational: an association, not proof that walking prevented it.

A damaged meniscus doesn't need surgery to matter. In a US study of adults aged 50 to 79 at high risk of knee OA, whose knees had never had surgery, meniscal damage was present at the start in 54% of knees that went on to develop osteoarthritis on X-ray within 30 months — versus 18% of knees that didn't. After accounting for age, s*x, BMI, physical activity and knee alignment, that was about 5.7 times the odds.

Knee OA reaches into working life. In southern Sweden, 15,345 working-age adults (16–64) with doctor-diagnosed knee OA were compared with the general working-age population in 2009. They were about twice as likely to have taken sick leave that year (1.82 times for women, 2.03 times for men) and averaged 87 sick days a year versus 57, age- and s*x-standardised. Knee pain in older Americans became more common over two decades — and not only because people got older and heavier. In national NHANES surveys of adults aged 60 to 74, knee pain on most days rose by about 65% from 1974 to 1994 in most groups studied, after accounting for age and BMI. In the Framingham study of adults 70 and over, symptomatic knee OA roughly doubled in women and tripled in men across about 20 years (1983–2005), also after adjusting for age and BMI — while knee OA seen on X-ray did not rise. Obesity explained only part of the increase.

Knee arthroscopy for degenerative knee disease is performed more than two million times a year worldwide, by one estimate. In 2017, an international guideline panel publishing in The BMJ, drawing on 13 randomised trials, made a strong recommendation against arthroscopy in nearly all patients with degenerative knee disease — with or without osteoarthritis on imaging.

Knee and hip osteoarthritis can follow people to bed. Among 300 US veterans with knee or hip OA enrolled in a clinical trial, 53% screened positive for symptoms of insomnia and 66% for symptoms of obstructive sleep apnea on standard questionnaires. Most of those with insomnia symptoms had no insomnia diagnosis in their medical record — only 22% did.
