Before and after: generator guarding.

BEFORE: This generator already had a guard on it. That was the problem. It looked covered. The existing guard had openings large enough for a person to reach in and contact the rotating hazard inside the generator. A guard that does not close the gap is not protection, it is the appearance of protection, and it is the kind of condition that passes a walk-by and fails an inspection.

AFTER: The original noncompliant guard was removed and a BCG custom designed guard was installed in its place. The new guard encases the entire face of the generator, preventing access to the rotating hazard.

This is precisely the condition a safety gauge catches on an inspection walk: an opening too large, or a guard sitting too close to the moving part.
