⚕️Behavioral health integration in primary care runs along a spectrum.

Coordinated care, colocated care, and fully integrated care each ask something different of the team.

At the integrated end sit the Collaborative Care Model and Primary Care Behavioral Health. Both rely on repeated, standardized assessment to give the team one objective read on a patient's progress.

A new article on the Creyos blog covers how each model runs and where cognitive assessment fits in.

📄 A new study found that remote cognitive assessment with Creyos is feasible in patients undergoing open heart surgery.

Co-authored by Creyos Staff Scientist Sydni Paleczny and Chief Scientific Officer Adrian Owen, the study found that:
➡️ 77% of the patients tested before their operation, and 66.7% of patients tested one month after, showed marked impairment on at least one cognitive task
➡️ Verbal processing was the most affected domain

Neuropsychological complications follow roughly half of cardiac surgeries. This study points to a way of measuring them that does not require patients to travel for assessment.

🧠 The lifetime risk of dementia for Americans over 55 is now estimated at 42%. Annual cases are projected to pass 1 million by 2060.

The SAGE is a free, self-administered screener developed at Ohio State. Patients answer 12 questions on paper, usually in 10 to 15 minutes. How well it performs depends on what you are looking for.

Our article looks at what it measures, what the evidence supports for MCI versus established dementia, and how it compares to the other screeners in common use.

💬 Psychiatrist Dr. Rebecca Reyes spent 20 years refining a workflow that worked. When digital assessment tools arrived, she saw little reason to change it.

Psychiatric assessment leans on the patient interview more than almost any other specialty. She worried a screen in the room would come between her and the people she was treating.

Her article on the Creyos blog covers what changed her mind, where objective measurement ended up fitting across intake, follow-up, and medication management, and what it took to get her staff and patients on board.

📄 Last year, Claremedica replaced paper-based cognitive screening with Creyos across more than 35 Florida sites.

Claremedica is a value-based senior primary care organization serving more than 34,000 Medicare Advantage patients, with a care model built around early detection.

Seven months in, more than 90% of the patients they newly identified with cognitive impairment were in the early or mild stages. That is the window where proactive care planning is still possible.

They are now tracking how earlier identification affects falls, avoidable hospitalizations, and total cost of care.

🧠 The Garrison Institute on Aging Memory Clinic at Texas Tech University Health Sciences Center serves more than 40 counties across 4 states.

Many communities have no access to cognitive screening and patients tend to reach care only once a caregiver is in crisis.

So clinic director Dr. Jonathan Singer took the screening to them, setting up in public libraries, senior centers, and community halls with Creyos on an iPad, run by staff with no specialized testing training.

In about a year his team screened more than 600 people, and roughly 1 in 4 were flagged for cognitive impairment, most at an early stage.

The next question his team is working on is what happens after a positive screen, and what stops people from acting on one.

🧠 Dementia care is outgrowing specialist capacity. The patients who stand to gain most from early intervention are often the ones waiting longest in the referral queue.

Some health systems have started moving objective cognitive assessment into primary care instead.

Our new white paper looks at how that redesign works in practice, including how one multi-state network rebuilt its cognitive screening program.

🧠 ADHD referrals keep climbing, and the diagnosis still rests largely on self-report from interviews, developmental history, and rating scales.

Objective testing can add a measurable layer, but different tools measure different things. Motion tracking captures observable movement. Cognitive assessment profiles working memory, executive function, and inhibition.

Our latest article covers what the evidence supports for each, and where objective data fits in an ADHD evaluation.

💬 The clinical interview is the foundation of psychiatric assessment, and psychiatrist Dr. Rebecca Reyes has spent over 20 years refining hers.

But what the interview was never designed to do is measure.

Subtle shifts in attention, memory, and executive function can sit below what a conversation will surface, even with a patient the clinician knows well.

In our latest article, she maps where objective cognitive measurement fits alongside the interview, and shares two cases where the data changed her decision.

🧠 55% of primary care physicians say their communities do not have enough dementia care specialists.

When a screen comes back abnormal, the next step can feel like it belongs somewhere else. Meanwhile medications go unreviewed, safety risks evolve, and the family keeps making daily decisions alone.

Dr. Anthony Zizza, a board-certified geriatrician and Chief Medical Officer at Element Care, wrote a new article on what primary care can do in the first 30 days after an abnormal cognitive screen.
