LIFTING PLAN – TRANSFORMER HANDLING OPERATION
A detailed lifting plan has been prepared for the safe handling and unloading of a 10-ton transformer using a 100-ton XCMG QY100K mobile crane.
The lifting operation will be performed at a working radius of 14.0 meters with the crane configured on fully extended outriggers over level and stable ground conditions. According to the crane load chart, the available lifting capacity at the specified radius is 17.7 tons, which provides an adequate safety margin for the planned load.
The transformer will be lifted using a certified 4-leg chain sling bridle arrangement. All rigging accessories, including the master link, chain slings, shackles, and swivel hook, have been selected in accordance with the required Safe Working Load (SWL) and inspected prior to the operation.
:
• Crane Model: XCMG QY100K
• Crane Capacity: 100 Ton
• Boom Length: 26.0 m
• Operating Radius: 14.0 m
• Crane Capacity at Radius: 17.7 Ton
• Load Weight: 10 Ton Transformer
• Lifting Arrangement: 4-Leg Chain Sling Bridle Hitch
• Utilization of Crane Capacity: 56.49%
📌 Safety Controls Implemented:
✔ Fully extended outriggers deployed
✔ Ground stability verified before setup
✔ Certified lifting gears utilized
✔ Load balancing confirmed before hoisting
✔ Controlled lifting to avoid shock loading
✔ Dedicated banksman and lifting team assigned
✔ Operation conducted under approved lifting plan and risk assessment
The lifting activity shall be executed in compliance with site safety requirements, lifting procedures, and international safe lifting practices to ensure a controlled and incident-free operation.

A Lost Time Injury (LTI) is a workplace injury or illness that causes an employee to be unable to perform their normal job duties for at least one full working day or shift after the incident.
It is an important safety performance indicator used in occupational health and safety management systems.

An incident becomes an LTI when:
🔷The worker is injured or becomes ill due to a work-related activity.
🔷The injury requires medical treatment or recovery time.
🔷The employee cannot return to normal work on the next scheduled working day or shift.
🔷The absence from work is officially recorded as lost work time.

The following workplace incidents may result in an LTI:
📌Fractures or broken bones
📌Serious cuts requiring stitches
📌Burns requiring recovery time
📌Sprains and strains
📌Head injuries
📌Eye injuries
📌Chemical exposure injuries
📌Slip, trip, and fall injuries
📌Machinery-related injuries

🔶An LTI is more serious than a simple first aid case.
🔶Restricted duties or light-duty work may not always be classified as LTI depending on company policy and legal requirements.
🔶Fatalities are recorded separately but are considered severe reportable incidents.
🔶Proper investigation and reporting of LTIs are essential to prevent recurrence.

A 🔥🧯contains: -

- an extinguishing agent (to suppress the fire) and a pressurized gas (to expel the agent).
The exact contents depend entirely on the specific type of fire the extinguisher is designed to fight.

Common fire extinguisher ingredients include:
Dry Chemical (e.g., ABC extinguishers):
Typically contains monoammonium phosphate or sodium bicarbonate. These fine, yellow or white powders work by coating the fire to smother it and interrupting the chemical reaction.

Carbon Dioxide (\(CO_{2}\)):
Contains pure, highly pressurized carbon dioxide gas. It works by displacing oxygen and cooling the fire, leaving no messy residue.

Foam:
Uses water mixed with foaming agents (like AFFF - aqueous film-forming foam).
It spreads a film over burning liquids to cut off the oxygen supply.

Water: Contains pressurized water with or without additives.
It cools the burning material but should never be used on electrical or grease fires.

Wet Chemical (e.g., Class K extinguishers): Contains potassium-based compounds like potassium acetate. When sprayed on cooking oils, it reacts to create a soapy, cooling barrier.

Pressurized Gas:
The agent is propelled by a compressed gas, most commonly nitrogen or argon.
