Pressure injuries rarely begin when skin breaks down. They begin much earlier — with reduced mobility, compromised tissue tolerance, aging physiology, and small clinical changes that often remain invisible before they become visible.
That is why prevention is one of healthcare’s most important disciplines.
As healthcare systems face increasing complexity and aging populations, pressure injury prevention is becoming more than a bedside intervention. It is evolving into a broader conversation about how we identify risk, protect dignity, and design care systems that support both patients and clinicians.
Nurses remain central to this process.
Their continuous presence at the bedside allows early recognition of subtle changes in tissue condition, mobility, comfort, and overall patient status. Yet clinical reality is challenging. Repositioning schedules can be difficult to maintain consistently in high-demand environments, and prevention strategies must increasingly adapt to individual patient needs.
Current international guidance emphasizes that no support surface completely replaces repositioning; prevention requires individualized care strategies supported by assessment and clinical judgment.

Manual repositioning every 2–3 hours sounds correct on paper.
