A small electrical stimulus at the lower leg produced measurable changes in cortical oxygenation after stroke. 🩸⚡

In the Phase 1 RETRAIN study, Dutta et al. (2026) studied 18 ischaemic stroke survivors receiving low-frequency neuromuscular electrical stimulation while NIRSport2 fNIRS monitored frontal and sensorimotor haemodynamics.

Cortical oxygenation increased with stimulation intensity, with larger responses in upright postures and participants with larger infarcts. By combining fNIRS with CO₂ and blood-pressure monitoring and cardiovascular modelling, the researchers could also investigate the systemic mechanisms behind these changes.

These early findings support further research into wearable electrical stimulation as a non-invasive addition to stroke rehabilitation.

How do you train for a movement that is too demanding and too risky to repeat indefinitely? 🎿✨

Yan et al. (2026) used fNIRS to measure cortical haemodynamic responses while 27 elite athletes performed action observation, motor imagery, or both simultaneously.

After correction for multiple comparisons, significant activation was found only when athletes combined observation(AO) with imagery (MI), particularly in precentral, postcentral, and middle frontal regions. The combined condition also generally produced stronger responses across frontoparietal areas associated with motor representation and visuomotor integration.

The results suggest AO + MI could be a promising way to investigate and potentially support off-snow cognitive training, while showing the versatility of fNIRS for studying complex motor processes in highly trained populations.

What makes an fNIRS or DOT experiment feasible? 🔍📈

Bulger et al. (2026) tackle this question before data collection, introducing a computational framework that predicts statistical power while accounting for cortical source properties, measurement configuration and experimental design.

The work shows how depth, source size and separation, cortical location and channel density interact with sample size and block count to influence detectability. Validated using experimental NIRSport2 high-density DOT data, the approach turns complex physical constraints into practical guidance researchers can use when planning studies.

It is a useful contribution towards designing more efficient, appropriately powered, and reproducible optical neuroimaging research.
