Broken Cameras, Missing Footage, and No One in Charge: Inside the Jail Failures Before Epstein’s Death

A sworn federal interview obtained from the Department of Justice’s Office of the Inspector General reveals deep and unresolved failures in the surveillance system at the Metropolitan Correctional Center (MCC) in New York in the days leading up to the death of Jeffrey Epstein — including cameras that were not recording, missing video footage, delayed repairs, and a stunning lack of accountability .

The interview, conducted under oath in April 2022, captures hours of questioning of a Federal Bureau of Prisons electronics technician responsible for the jail’s camera systems. His testimony paints a picture of chronic dysfunction inside one of the federal system’s most sensitive facilities at a critical moment.

Cameras That Were Not Recording

According to the testimony, MCC’s surveillance system was experiencing serious failures in early August 2019. Cameras were reportedly not recording, and staff were unable to retrieve previously recorded video when they tried to do so — a problem that came to light just days before Epstein was found dead in his cell .

The technician confirmed that officials alerted him to the problem and that attempts were made to determine why footage could not be pulled. However, he repeatedly stated he could not say with certainty which cameras were affected or how extensive the failure was.

A Key Contradiction Over Repairs

One of the most significant red flags centers on whether the system was fixed before Epstein’s death.

A written memo from an SIS lieutenant states that the technician said he repaired the camera system on Friday, August 9, 2019 — the day before Epstein died. Under oath, the technician denied this account, saying he did not believe he repaired the system that day and that the claim did not align with his recollection or the access he had at the time .

Investigators repeatedly pressed him to reconcile the discrepancy. It remained unresolved.

Locked Out During a Crisis

The technician also testified that he was unable to access critical surveillance rooms, including the SIS room, because he did not have keys and required staff were not present. Keys were kept locked behind glass, and no one was available to grant him entry, he said.

At the same time, supervisors and command staff did have keys — raising serious questions about why access was not facilitated during an acknowledged system failure .

Problems That Existed “Forever”

Perhaps most striking was the technician’s repeated assertion that camera problems at MCC were not new. He described the video monitoring room as having issues “forever” and said systems frequently worked intermittently, long before August 2019 .

Despite these known issues, the system continued to operate without full remediation during a period when Epstein was under heightened scrutiny following a prior su***de attempt.

A Surveillance Upgrade That Never Arrived

The interview also exposes a deeply mismanaged camera upgrade project approved in 2018.

Hundreds of new cameras were ordered, but according to the technician:
• Equipment arrived in pieces
• Some components were misplaced or stored off-site
• Many cameras were never installed
• Others were allegedly delayed because of approval, compatibility, or storage issues

He acknowledged telling the contractor to delay shipping cameras at one point, citing the lack of secure storage and unresolved approval issues. Contractors later indicated they only received final clearance to ship cameras after Epstein’s death .

No One Clearly in Charge

Throughout the interview, a consistent theme emerges: no single person appeared to be responsible for ensuring MCC’s surveillance system was functional.

The technician emphasized that he lacked authority to approve equipment, sign for deliveries, or make final decisions. Responsibility appeared fragmented among multiple offices and officials, many of whom are unnamed in the transcript or described as absent during key moments .

Lingering Questions

The sworn testimony does not accuse any individual of wrongdoing. But it documents a system marked by contradictions, delays, missing equipment, and access failures — all at a time when the jail was responsible for safeguarding one of the most high-profile detainees in the country.

More than five years later, fundamental questions remain unanswered:
Who knew the cameras were failing?
Who had the authority to fix them?
And why, when the stakes were highest, did the system appear to fail so completely?

The Inspector General’s own record suggests the answers may lie not in a single act, but in a cascade of institutional breakdowns that no one ever fully owned .
