A clinic asks whether the plasma step puts heat into the skin. Physically it does not — and the reason decides which numbers belong in the protocol.

𝗧𝘄𝗼 𝘁𝗲𝗺𝗽𝗲𝗿𝗮𝘁𝘂𝗿𝗲𝘀 𝗶𝗻𝘀𝗶𝗱𝗲 𝘁𝗵𝗲 𝘀𝗮𝗺𝗲 𝗴𝗮𝘀

Cold atmospheric plasma is a non-equilibrium state. Free electrons are heated far more efficiently than the heavy particles around them, so the electrons run very hot while ions and neutral gas molecules stay near ambient temperature. The heavy particles carry almost all of the mass, so the gas that meets the skin is not hot: published dermatology reviews report treated skin staying close to body temperature during CAP application (about 37–38 °C), below the 40 °C ceiling defined for skin-safe operation (Khalaf et al., Eur J Med Res 2024;29:487).

𝗦𝗼 𝘁𝗵𝗲 𝘄𝗼𝗿𝗸𝗶𝗻𝗴 𝗮𝗴𝗲𝗻𝘁 𝗶𝘀 𝗰𝗵𝗲𝗺𝗶𝘀𝘁𝗿𝘆, 𝗻𝗼𝘁 𝗵𝗲𝗮𝘁

Those high-energy electrons drive excitation, ionisation and dissociation at low gas temperature, producing reactive oxygen and nitrogen species, charged particles and local electric fields. That is why the barrier effect is described as transient permeabilisation rather than a thermal effect.

𝗪𝗵𝗶𝗰𝗵 𝗺𝗮𝗸𝗲𝘀 𝗴𝗲𝗼𝗺𝗲𝘁𝗿𝘆 𝗮𝗻𝗱 𝘁𝗶𝗺𝗲 𝘁𝗵𝗲 𝗱𝗼𝘀𝗲

In plasma physics measurements on pure water, the formation rate of H₂O₂ and OH radicals dropped by orders of magnitude as the nozzle-to-surface gap widened from 0.5 to 5.5 cm, while nitrite and nitrate changed by about one order (Sergeichev et al., Front Phys 2021;8:614684 — a model liquid, not skin). Distance and exposure time per area are protocol entries, not operator habits.

On the mid-forehead, MRI measurements put roughly 5 mm between the skin surface and the outer cortex of the frontal bone. Several facial depth options reach further than that.

𝗕𝗼𝗻𝗲 𝗶𝘀 𝗮𝗻 𝗮𝗰𝗼𝘂𝘀𝘁𝗶𝗰 𝗯𝗼𝘂𝗻𝗱𝗮𝗿𝘆, 𝗻𝗼𝘁 𝗮 𝗯𝗮𝗰𝗸𝗱𝗿𝗼𝗽

Soft tissue and cortical bone sit far apart acoustically — published measurements cite about 1.62 MRayl for soft tissue against 6.27 MRayl for cortical bone (Pham et al., Proc Inst Mech Eng H 2022;236(9):1403–1413). At that mismatch, a substantial part of the incident energy does not continue into bone: some is absorbed right at the surface, some is reflected back into the tissue just above it. Therapeutic ultrasound literature, working at far higher intensities, names the consequence plainly — the periosteum, the most innervated structure of mature bone, is heated by that absorbed and reflected energy (Scipione et al., Semin Intervent Radiol 2018;35(4):261–267).

𝗛𝗼𝘄 𝗺𝘂𝗰𝗵 𝗿𝗼𝗼𝗺 𝘁𝗵𝗲 𝗮𝗻𝗮𝘁𝗼𝗺𝘆 𝗮𝗰𝘁𝘂𝗮𝗹𝗹𝘆 𝗴𝗶𝘃𝗲𝘀 𝘆𝗼𝘂

An MRI study of 160 adults measured forehead soft tissue from skin to the external cortical lamina: about 5.7 mm midline in men and 5.3 mm in women, 7.4 and 6.8 mm laterally, with thinning after the fifth decade (Cakmakoglu et al., Aesthetic Plast Surg 2025;49(19):5275–5283). The depth budget is regional and individual, not universal.

Cold plasma is usually described as the step that "prepares the skin." Published work on the technology is more specific: what it opens, it also closes — within minutes.

𝗪𝗵𝗮𝘁 𝘁𝗵𝗲 𝗯𝗮𝗿𝗿𝗶𝗲𝗿 𝘀𝘁𝗲𝗽 𝗮𝗰𝘁𝘂𝗮𝗹𝗹𝘆 𝗶𝘀

In published dielectric barrier discharge research on human skin ex vivo, plasma permeabilises the stratum corneum across the treated area. Permeabilisation was tracked as a change in transepithelial electrical resistance, and Franz-cell permeation with differently sized hydrophilic particles was used to estimate pore characteristics. The barrier is not removed — it is transiently made passable.

𝗧𝗵𝗲 𝘄𝗶𝗻𝗱𝗼𝘄 𝗶𝘀 𝘀𝗵𝗼𝗿𝘁, 𝗮𝗻𝗱 𝗶𝘁 𝗵𝗮𝘀 𝗯𝗲𝗲𝗻 𝗺𝗲𝗮𝘀𝘂𝗿𝗲𝗱

On human skin explants treated with a kHz plasma jet, surface pH, wettability and barrier readings all shifted and returned to pre-exposure levels within roughly 20 minutes. Inside that window, caffeine reached the receptor chamber about 30 minutes after topical application, versus about 8 hours on untreated controls; hyaluronic acid was distributed across the explant after 1 hour instead of remaining in the upper layer (Vijayarangan et al., Front Phys 2023;11:1173349).

Published reviews of microfocused ultrasound describe the same variable on the treatment side: pressure applied at the provider's discretion, with coupling maintained, can shift the effective treatment depth by up to about 1.5 mm.

Within a single face, published ultrasound measurements place the dermis of the mouth corner and the dermis of the lateral forehead at opposite ends of the range — and the plane you intend to treat moves with them.

𝗪𝗵𝗮𝘁 𝗺𝗲𝗮𝘀𝘂𝗿𝗲𝗺𝗲𝗻𝘁 𝗼𝗳 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻 𝗮𝗰𝘁𝘂𝗮𝗹𝗹𝘆 𝘀𝗵𝗼𝘄𝘀

High-frequency ultrasound of eight facial sites in 118 healthy adults reported mean dermal thickness from about 0.58 mm (eyelid, women) to 1.17 mm (forehead, men) — roughly a twofold spread across sites and s*xes (Meng et al., BMC Med Imaging 2022;22:113). A separate eight-site study found the thickest dermis at the corner of the mouth and the thinnest at the lateral forehead, with full-skin and dermal thickness lower in women than in men (Jeong et al., Skin Res Technol 2023;29(8):e13426).

𝗪𝗵𝗶𝗰𝗵 𝘃𝗮𝗿𝗶𝗮𝗯𝗹𝗲𝘀 𝗵𝗼𝗹𝗱 𝘂𝗽 — 𝗮𝗻𝗱 𝗼𝗻𝗲 𝘁𝗵𝗮𝘁 𝗱𝗼𝗲𝘀𝗻'𝘁

Site and s*x differences appear in both datasets, and age-related dermal thinning was reported in women at the forehead, glabella, zygoma and submandibular sites. BMI is less consistent: one study found a neck correlation in women, the other no significant correlation with BMI at all. Taken together, that makes site the variable worth planning around — with the absolute millimetre values read as cohort reference points, not universal constants.

A skin booster placed too superficially stays in the epidermis; placed too deep it passes the papillary layer you were aiming for. So what actually decides where it lands?

In needle-free liquid jet injection, the depth and lateral spread of the jet in skin are governed by nozzle geometry and jet velocity together. Schramm-Baxter and Mitragotri (J Control Release 2004;97(3):527–535) varied both across a wide experimental range and found that the geometry of pe*******on and dispersion collapsed onto a single parameter — jet power. Two systems showing a similar setting on screen are not delivering the same thing if the nozzle differs.

Two patients with the same thigh circumference can have very different tissue architecture underneath — and the same plan will not describe both.

𝗪𝗵𝗮𝘁 𝘁𝗵𝗲 𝗵𝗶𝘀𝘁𝗼𝗹𝗼𝗴𝘆 𝗮𝗰𝘁𝘂𝗮𝗹𝗹𝘆 𝘀𝗵𝗼𝘄𝘀

The classic anatomic work on subdermal architecture (Nürnberger & Müller, J Dermatol Surg Oncol 1978;4(3):221–229) reported that the connective tissue between dermis and subcutis is organised differently by s*x: radial, arch-like chambers in women, versus tangential, criss-crossing units in men. A later biomechanical study of gluteal tissue in 20 body donors (Rudolph et al., Plast Reconstr Surg 2019;143(4):1077–1086) quantified it — subdermal fat lobules were more numerous in male donors (10.05 ± 2.3) than female donors (7.51 ± 2.7; p = 0.003), and the force needed to break the septa was higher in males (38.46 ± 26.3 N) than females (23.26 ± 10.2 N; p = 0.021). The authors describe the subdermal junction as a balance between containment and extrusion.

The standard operating protocol for energy-based devices (Mysore et al., J Cosmet Dermatol 2024;23(12):3809–3820) was written for exactly this situation: "treating doctor and operating doctor may be different." Its asks are unglamorous — parameters set from the doctor's written instructions, a signed pre-procedure checklist for contraindications, consent dated before every repeated session, final parameters entered in the record, and an experienced consultant available to supervise.

Litigation reviews in cutaneous laser surgery point the same way: 175 cases from 1999–2012, 42.9% involving a nonphysician operator and 64% of those outside a traditional medical setting (Jalian et al., JAMA Dermatol 2014;150(4):407–411); nonphysician operators in 71% of 69 claims from 2012–2020 (Khalifian et al., Dermatol Surg 2022;48(3):315–319). Those are laser data — not microwave, not about HIRONIC devices, and not device-specific at all. The transferable signal is supervision and documentation.

A patient books a body contouring session and mentions a pacemaker only after lying down on the treatment table. What should happen next should already be written down.

𝗪𝗵𝘆 𝗮𝗰𝘁𝗶𝘃𝗲 𝗶𝗺𝗽𝗹𝗮𝗻𝘁𝘀 𝗮𝗿𝗲 𝘁𝗵𝗲𝗶𝗿 𝗼𝘄𝗻 𝗰𝗮𝘁𝗲𝗴𝗼𝗿𝘆

For most screening questions, the answer changes a setting. For an active implanted electronic device, it changes whether the session happens at all. A review of physiotherapy practice in patients with cardiac rhythm devices (Digby et al., Europace 2009;11(7):850–859) concluded that diathermy — the modality that heats tissue with shortwave or microwave energy — is best avoided in these patients, and noted that device manufacturers advise against it. The concern is not the dose on the skin. It is energy coupling into an implanted system.

Manufacturers love to advertise energy output — joules, watts, peak power. But energy alone doesn't tell you what actually happens inside the tissue.
