~Dr.Abu-Abdallah PT,MNSP,Msc(inview) Orthopeadic and sports:
*MRTB, the Nigerian Physiotherapist's underutilized weapon for self actualization*

The Medical Rehabilitation Therapist (Registration) Board (MRTB) is the regulatory agency for the practice and training of medical rehabilitation professions in Nigeria. Medical rehabilitation professions under the MRTB's scope of influence include Physiotherapy, Occupational Therapy, Prothesis and Orthosis , Clinical Audiology and Speech Therapy. Of all these professions, Physiotherapy remains the most established product of MRTB and contributes the largest percentage of her workforce. In other words, MRTB is controlled by Physiotherapists.

MRTB has the role of determining the standard of Physiotherapy practice and training in Nigeria. This means that MRTB being responsible for licensing Physiotherapy practice is supposed to be the most important factor in deciding the fate of every Physiotherapist in Nigeria. The number of Physios employed, the quality of training given to aspiring Physios and the quality/cost of services delivered in public institutions as well as in private ones are all supposed to be within the infuence of the MRTB. Unfortunately, though I stand to be corrected, the MRTB has only been effective in issuance of licence, conduction of induction and organization of conferences leaving the other aspects to suffer.

The failure of the MRTB to live up to her duties is one of the reasons Physiotherapy seems to be begging for recognition in the current health system in the country. It is no gainsaying that most Physiotherapy departments in federal and state health institutions are in sorry states with little or no space for offices not to talk of availability of equipments. These departments are grossly understaffed and the few physios around are masters in improvisation (an outstanding traits all Nigerian Physios are not duly appreciated for). In some institutions, Physios are not rightly given their place of pride among the clinical staff even when they too wear white coat and document on patient's folder in the wards.

In our institutions of learning even among the foremost ones like the one I graduated from, Physiotherapy lecturers lack individual offices and no adequate class rooms and laboratories can be classified Physiotherapy specific. In recent times, in a bid to reduce the pride of Physios and prove that Physios are inadequately trained to work independently of the Medical doctors, some medical schools (orchestrated by our sister profession who claim to be 'Lord of the health sector') have now separated the Physios from the other medical students in course like anatomy, physiology, pharmacology and pathology which before were a common class for both Physios and medical doctors in training.

In all these happenings, the MRTB is believed to always be going for accreditation exercises and deeming these conditions satisfactory for granting accreditations to these institutions. All these raise questions in my mind (I don't know if it does too in your mind); 'what are the minimum requirements in terms of equipment and staffing for granting of accreditation for the practice and training of Physiotherapy in Nigeria?' and ' Are accreditation inspections often really carried out in reality or its an 'audio thing'?'.

I am of the opinion that the MRTB is one potent channel through which the Nigerian Physios can take their fight to the powers that be. If MRTB becomes effective and strict in matters concerning accreditation of clinics and training institutions, unemployment and sidelining of the Physios can be greatly reduced. Imagine what would happen if all the Physiotherapy clinics and departments in a state loose their accreditation for poor funding, understaffing and lack of equipments?, Don't you think this can cause more attention to be paid to Physiotherapy?. Imagine what would happen to the Physiotherapy profession if the MRTB actually set up committees and fully empower them to check quackery in all states of the federation rather just posting names from Lagos without anything to empower such committees?.

I believ

e our struggle for self actualization is a good one but I think we can achieve it faster if we add the MRTB as our secret landmine to the NSP warheads.

This is just my opinion.

WORLD PHYSIOTHERAPY DAY

Dear Colleagues

I am very delighted to wish you a happy World Physiotherapy Day. This yearโ€™s celebration coincides with the 60th Anniversary of the Nigeria Society of Physiotherapy and therefore gives us a lot more to celebrate, but more importantly to make reflections about the state of our profession.

Physiotherapists in Nigeria have distinguished themselves in putting people back to work and daily lives and have contributed towards reducing and eliminating the chronic pain of many Nigerians. This indeed calls for celebration of all Physiotherapists contributing towards uplifting the image of the profession in various areas of endeavor. The upgrade of the Bachelor of Physiotherapy (BPT) to Doctor of Physiotherapy (DPT) is also worth celebrating as it sets a stronger foundation for specialization and professional autonomy. The establishment of the Postgraduate Physiotherapy College, which is registered and recognized by relevant government agencies, gives Physiotherapists in Nigeria the opportunity to develop clinical skills and competencies required to give best services to the people of Nigeria and beyond. The development of Clinical Practice Guidelines by NSP Specialty Groups and partnership with eight hospitals to establish Centers of Excellence in eight Physiotherapy specialty areas are also important aspects to celebrate as we celebrate this yearโ€™s World Physiotherapy Day. These achievements would not have come to fruition without the efforts and contributions of Physiotherapists in Nigeria over many years.

Just as we are celebrating our collective achievements, it is a good time to make reflections about the state of our profession. How are our ethical conducts in the practice of Physiotherapy? How do we ensure the Postgraduate Physiotherapy College gives us the competencies required to change our practice and help our patients better? How do we push for further institutionalization of the College through bills in the National Assembly and other processes? How do we sensitize government agencies to employ Physiotherapists to provide more services to Nigerians while at the same time engage our teeming youths? How do we implement aspects of the Scheme of Service that ensure capacity building as we move across ranks? How do we establish good governance in the running of our dear Society, the NSP? How do we participate in policy making and implementation to ensure our interests are captured in policy documents?

These questions can only be comprehensively answered if we put our heads together and chat a way forward for our Profession. I would like to encourage you to attend the 59th Annual Scientific Conference and 60th Anniversary of the Nigeria Society of Physiotherapy in October where important decisions will be taken to answer the questions raised above. The NSP 5-Year Strategic Plan, Reviewed Ethics and Professional Practice Guideline, Ordinance of the Postgraduate Physiotherapy College and Reviewed NSP Constitution are some of the important documents that will be considered at the Conference/AGM.

I wish to finally and most importantly call on all Nigerian Physiotherapists to give unity a chance. Our individual capacities put together is much greater than any individual or group's capacity in tackling the myriad of challenges we are facing. I pray that the 60th Anniversary of the Nigeria Society of Physiotherapy will witness a very big boost with the coming together of all Physiotherapists in Nigeria.

Thank you and God bless you all.

Rufai Ikechukwu Gbolahon Ahmad
President, NSP.
