ACCIDENT REPORT AT LAI CAMP
For the interest of the General Public curios to know regarding an incident that occurred on Sunday which got public attention on social media , this issues has been resolved and amicably came to an understanding between the company and the deceased family .
We give credit to our peace loving people of Meki, KKR and Lower Mendi Generally, the Peace and Good order community and of course the hard working Police officers.

Subject: Incident Report Regarding the Death of Late Joshua (Security Guard)
Leadership and Dialogue
Mr. Oken Molumi and Mr. Mike Tunu demonstrated commendable leadership by facilitating mature dialogue with the family of the deceased, Late Joshua, on behalf of COL. While Mr. Molumi expressed his intention to assist in any way possible, he made it clear that he would not take responsibility for the incident itself.

This report provides a comprehensive account of the circumstances surrounding the fatal accident involving Late Joshua, who served as a security guard.

Mr. Egypt - Machine Operator Statement
Time of Incident: Approximately 2:00 PM
I was preparing the pre-coat for sealing the work STP. After completing my tasks, I left the machine in its usual location and went off to Batera for about three hours. Upon my return, I learned that the machine had exited the camp and became involved in a tragic accident.
Grease Boy (Name Not Provided)
I was instructed by Mr. Egypt to grease the machine. After I completed this task, I left the site. Later, I discovered that the machine had departed from the camp and was involved in an accident.

Time of Work: 6:00 AM to 4:30 PM
I arrived on-site and noticed that we had a five-man team working. Late Joshua replaced me for his shift as my team left for home.

Mr. Mark Karia - Safety Officer Statement
I departed Oiyarip camp and arrived at Wara Kombu camp around 5 PM. At that time, I noticed the machine parked alongside the main road corridor. Believing it was Mr. Egypt operating it after hours, I instructed him to take the machine back inside the camp without realizing that Late Joshua was actually in control of the machine.
Civilian Eyewitness
An unidentified civilian observed Late Joshua operating the machine, inviting a bystander to join him in going to get Bui-Smoke. This decision ultimately led to the accident.
Additional Eyewitness - Simbu Paul
While charging my boom box in the camp, I observed the machine’s engine running and Late Joshua standing outside the machine, while Max, the grease boy, was sitting on it.

There is evidence of unauthorized individuals tampering with the machinery, highlighting a significant breakdown in security and control protocols at the site.
