𝐐𝐮𝐚𝐥𝐢𝐟𝐢𝐞𝐝 𝐯𝐨𝐭𝐞𝐫𝐬 are members residing in the barangays of 𝐃𝐢𝐬𝐭𝐫𝐢𝐜𝐭 3, 𝐃𝐢𝐬𝐭𝐫𝐢𝐜𝐭 4, 𝐂𝐚𝐥𝐢𝐩𝐚𝐭-𝐚𝐧, 𝐖𝐚𝐬𝐡𝐢𝐧𝐠𝐭𝐨𝐧, 𝐈𝐧𝐭𝐞𝐫𝐢𝐨𝐫, 𝐃𝐨𝐫𝐨𝐧𝐠-𝐚𝐧-𝐃𝐚𝐩𝐥𝐢𝐚𝐧, 𝐁𝐚𝐠𝐚𝐜𝐚𝐲, 𝐋𝐮𝐧𝐚, 𝐁𝐚𝐫𝐭𝐨𝐥𝐚𝐛𝐚𝐜, 𝐚𝐧𝐝 𝐃𝐚𝐧𝐚𝐨.
𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬 𝐟𝐨𝐫 𝐅𝐢𝐥𝐢𝐧𝐠 𝐂𝐎𝐂:
• Personal Data Sheet with 2x2 photos with attached scholastic record (Original TOR/ Original or Authenticated College Diploma/ Special Order or CHED Certification )
• PSA Birth Certificate and Marriage Certificate (if applicable)
• Barangay Clearance and Residency Certification
• Police, NBI, MCTC, and RTC Clearances
• Statement of Assets, Liabilities and Net Worth (SALN)
• Medical Certificate issued by a Government Physician
• Certification certifying that the applicant and the spouse are not holding elective or appointive position in the government with a salary grade of SG16 or higher or equivalent as evidenced. (If the applicant is a retiree, certification from the agency)
• Clearance from the Finance Department together with the photocopy of the latest OEBR (Official Electric Bill Receipt) and Certification from the teller attesting that the applicant has no unsettled accounts with the cooperative.
• Identification Number (TIN) with Latest Community Tax
• Proof of attendance in at least two (2) Annual General Membership Assemblies (AGMA).
• Other certifications and documents required under TISELCO Election Guidelines.
