ASTRO and CANYON GUIDE harness safety alert: Replacement of the ventral D-ring

User feedback alerted us to premature wear of the screws on the gated ventral attachment point on ASTRO rope access harnesses manufactured before October 2023 (harnesses that were affected by our call for inspection in 2023, including CANYON GUIDE harnesses).

This wear can cause the ventral D-ring’s pin to release, and the attachment point to open, which could lead to serious injury or death. This is why we ask that you replace the metal D-ring and the screws on the gated ventral attachment point, upon receiving the replacement kit.

Mechanical Wedge Anchors in Brick Masonry — A Common but Serious Installation Mistake

Mechanical wedge anchors are expansion-type anchors.
They develop load resistance by creating radial pressure against the drilled hole wall when torque is applied.

This mechanism works effectively only in dense structural concrete.

WHAT HAPPENS IN REINFORCED CONCRETE?

• Expansion force distributes into high-compressive-strength material
• Reinforcement helps control cracking
• Tensile stresses are resisted by the concrete mass
• Load transfer is predictable with proper embedment

When installed correctly (correct hole size, cleaning, torque, edge distance), wedge anchors perform reliably in structural concrete.

WHAT HAPPENS IN BRICK MASONRY?

Brick behaves very differently.

• Brick is brittle
• Lower tensile capacity
• Mortar joints create weak planes
• Possible voids or inconsistent density
• No reinforcement to control cracking

When a wedge anchor expands inside brick:

• Radial pressure induces splitting forces
• Internal cracking may occur before visible damage
• Load may transfer into mortar instead of solid brick
• Pull-out capacity becomes unpredictable
• Failure can be sudden and brittle

An anchor that feels “tight” does not mean it is structurally safe.

WHY THIS MATTERS FOR SAFETY ANCHORS

Fall protection anchors are subjected to:

• Tensile loading
• Dynamic forces (fall arrest)
• Environmental exposure
• Long-term service conditions

Installing expansion anchors into brick masonry for life-safety applications significantly increases risk of cracking, pull-out, and substrate failure.

Professional Practice Reminder:

Before installing any mechanical anchor:
Confirm substrate type (do not rely on surface appearance).
Verify structural reinforced concrete.
Check embedment depth, spacing, and edge distance.
Follow manufacturer torque specifications.
Conduct proof load testing where required.

Expansion anchors rely on compressive confinement.
Brick masonry does not provide reliable confinement for this anchor type.

Hard to Differentiate Between RC Wall and Brick Wall During Safety Anchor Installation

Not all concrete-looking walls are structural.

During safety anchor installation, one of the most common risks is assuming that a plastered wall is reinforced concrete. In reality, many buildings use brick masonry infill walls with plaster finishing, which visually resemble structural concrete.

From the surface, both look similar.
Structurally, they behave very differently.

Reinforced Concrete (RC) Wall:
• Designed to carry structural loads
• Contains steel reinforcement
• Higher compressive and tensile resistance
• Suitable for fall arrest anchors when properly installed and verified

Brick Wall with Plaster Finish:
• Typically non-structural infill
• Lower compressive strength
• Brittle failure behaviour
• Expansion anchors can cause cracking or sudden pull-out under load

For fall protection systems, safety anchors are expected to resist significant tensile forces. Mechanical anchors rely heavily on the integrity of the base material. Installing into the wrong substrate can result in reduced load capacity — even if the anchor feels secure.

Before installation, proper verification is essential:
• Confirm structural drawings
• Verify substrate thickness
• Avoid installing into plaster layer
• Conduct proof load testing after installation

Surface appearance alone is not sufficient to confirm load-bearing capacity.

Anchor performance depends not only on the product — but on the substrate and installation quality.

Horizontal Lifeline System Inspection & Load Verification
(SS 570 & EN 795 Aligned)

Safety at height is not only about installation — it is about continuous engineering verification.

Our B2S Specialist team recently carried out a full Horizontal Lifeline (HLL) inspection and proof load verification on a metal roof system in alignment with:

• SS 570:2011 – Anchor devices and flexible horizontal lifeline systems (Singapore Standard)
• EN 795:2012 – Personal fall protection equipment – Anchor devices

Environmental exposure, thermal roof movement, corrosion, vibration and prior adjustments can significantly affect system performance over time.

A lifeline that appears visually intact does not automatically mean it remains compliant, correctly tensioned, or capable of performing under design load conditions.

Inspection & verification scope included:

✔ Detailed visual inspection of cable, terminations, brackets and intermediate supports
✔ Verification of base plate condition and fastener integrity
✔ Cable tension measurement and recalibration where required
✔ Proof load verification of anchor interface points
✔ Review of system alignment and structural interface condition

We ensure the system:

• Maintains correct cable tension within engineered parameters
• Performs under required load conditions
• Remains compliant with SS 570 / EN 795 requirements
• Is fit for continued use for personnel working at height

Horizontal lifeline systems are engineered safety components — not passive roof accessories.

Many assume that once an anchor is installed, it is ready for use.
In reality, visual checks alone are not enough.

In professional rope access operations, safety anchors should be proof load tested to verify:
✔ Correct installation into substrate
✔ Structural strength of base material
✔ Performance under working load conditions
