Researchers in Ethiopia have developed a new strategy for diagnosing tuberculosis (TB) in low-resource, high-incidence settings: Photographing film x-rays to create digital files and then feeding them to AI.

In a pilot study that included 498 such images, radiologists detected up to 99 cases indicative of TB, while an AI model detected 81 cases, noted lead authors Zerubabel Desita, MD, and Temesgen Tadesse, MD, of the University of Gondar, and colleagues.

“Successful mobile phone-based analog radiographic image analysis could represent an easily implementable tool for screening and reduce diagnostic barriers for people living in low-resource settings,” the group wrote. The study was published January 21 in Mayo Clinical Proceedings: Digital Health.

With an estimated 10.8 million people experiencing TB around the world in 2023 and 1.09 million deaths that year due to the disease, TB remains a major threat to global health. Previous studies have shown that AI models can improve the detection and diagnosis of TB on chest x-rays in areas where experienced radiologists are scarce, the authors explained. Yet these studies have focused on the analysis of digital images, which limits the technology’s applicability in settings where chest x-rays are available only in analog form, the researchers noted.

The group aimed to assess the diagnostic value of an AI model (qXR, Qure.ai) for TB using film x-rays photographed by mobile phones or digital cameras. The analysis used 498 images from patients seeking help for TB symptoms between January 2017 and March 2018 at health centers in Ethiopia and Guinea-Bissau who had a final diagnosis by clinical or laboratory tests.

Compared with assessments by two Ethiopian radiologists who identified 50 and 99 cases indicative of TB, the AI model identified 81. The overall area under the curve (AUC) for the model was 0.84 compared with laboratory-confirmed cases. The model's sensitivity was 76.5%, and its specificity was 85.9%, while radiologist A’s sensitivity was 64.7% and specificity was 91.9%. Radiologist B’s sensitivity was 76.5% and specificity was 82.3%, compared with laboratory-confirmed cases.

🩻AI predicts from lumbar spine x-rays
An model appears to be a practical and reliable tool for predicting osteoporosis from frontal lumbar spine x-rays, researchers in Japan have reported.

The finding is from a study involving 332 patient x-rays from which the model estimated bone mineral density, with its estimations correlating strongly with gold standard dual-energy x-ray absorptiometry ( ) scans, noted lead author Ryusei Inamori, a doctoral student at Tohoku University in Miyagi, Japan, and colleagues.

“[The software] may provide screening opportunities for patients in various medical settings and contribute to extending healthy life expectancy among the elderly through early detection,” the group wrote. The study was published in the January-March issue of the Journal of Clinical Densitometry.

Osteoporosis affects approximately 200 million people worldwide. DEXA scanners are available only in limited facilities, which can hinder early screening for the disease, the authors explained.

To date, several AI models have been developed that predict osteoporosis by estimating bone mineral density (BMD) from x-rays, yet these models have generally used data from geographically homogeneous populations, the group noted. One such model (DeepXray Spina, Alpha Intelligence Manifolds) was developed using approximately 15,000 DEXA scans from four medical institutions in Taiwan. For this study, the researchers tested its generalizability across a Japanese cohort.
