Mark Piehl, MD, MPH and Peter Antevy MD, FAEMS spoke to standing-room crowds at EMS World - Emergency Medical Services on "Rethinking Pediatric Trauma: Circulation Comes First” and in a second talk hosted by Will Heuser, "Drug Wars, Pediatric Edition: Little Patients, Big Controversies.” Both talks emphasized the importance of early transfusion for hemorrhagic shock, and the need to get blood products into the field to treat injured children as soon as possible. Mark, Peter, and Will addressed a number of other controversies including prehospital advanced airway management for kids, CPAP for respiratory failure, and Ketamine for refractory status epilepticus.

A 30-year-old woman in severe hemorrhagic shock from a ruptured ectopic pregnancy received 2 units of whole blood in approximately 15 minutes through a 20-gauge IV using LifeFlow. Her blood pressure and mental status improved during transport, helping stabilize her before arrival at the hospital for definitive care.

Pasadena Fire Department is already saving lives with its prehospital blood program. Recently, firefighter/paramedics used whole blood to treat a man who was struck by a vehicle—bringing critical blood products to the patient when every minute mattered.
