A visual acuity entry such as “20/20” or “20/40” only tells part of the story. Without correction status, testing distance, notation system, and other clinical context, the next person reading the chart may not know exactly what was measured.
This guide reviews Snellen, decimal and logMAR notation, pinhole documentation, near visual acuity, low vision charting, pediatric optotypes, and the details that help make VA records more consistent and useful across visits. It also covers pediatric documentation with LEA SYMBOLS® and HOTV, including why the optotype and testing distance should be recorded.
