The word creatinine is derived from creatine named after the Greek word kreas (โ€œmeatโ€) by chemist Michel Eugรจne Chevreul who first isolated creatine from beef.

Creatine and phosphocreatine maintain ATP concentrations in muscle and other tissues with high-energy demands. Muscles do not store large amounts of ATP directly, but phosphocreatine can instantly donate its phosphate to recharge spent ATP.

Creatinine, the spontaneous non-enzymatic degradation product of creatine/phosphocreatine, diffuses out of cells into circulation and is eliminated in urine. Clinical testing of creatinine levels in blood and urine is used to measure how well kidneys filter waste from the body.

Medication adherence in organ transplant refers to the recipient following agreed upon recommendations and instructions from healthcare professionals for taking immunosuppressive medications at the prescribed dosage and times with consistency and persistence.

Medication non-adherence (MNA) is estimated to occur in 22-68% of transplant recipients and is responsible for approximately 50% of late rejections and 15% of graft loss.

The ability to adhere to a consistent immunosuppressant regimen is impacted by a variety of factors including difficulty obtaining medications, insurance issues, disruptions in daily routine, poor social support and drug side effects.

โ€ข Immunosuppression of an aging immune system may exacerbate risks of malignancy, cardiovascular disease and diabetes through chronic low-grade inflammation and reduced tumor surveillance.

โ€ข Age-related metabolic changes increase adverse effects.

Important considerations
โ€ข Older donor kidneys may have higher rejection risk.

Our melanin has three main types in humans: eumelanin, pheomelanin and neuromelanin. In skin, eumelanin and pheomelanin are synthesized from the amino acid tyrosine via tyrosinase and they protect us from UV radiation.

Neuromelanin, a complex polymeric molecule containing eumelanin and pheomelanin, concentrates in the substantia nigra pars compacta brain region, central to movement and reward, within catecholaminergic neurons. Neuromelanin is thought to mainly form from oxidative polymerization of dopamine with incorporation of cysteine. By soaking up toxic substances like heavy metals, neuromelanin helps protect neurons.

Neuromelanin accumulates as we age in the brain area prone to degeneration in Parkinsonโ€™s disease and other neurodegenerative disorders. Neuromelanin-sensitive imaging signals are being explored as a biomarker of brain health.

Born on this day in 1859 was scientist ๐‰๐จ๐ก๐ง ๐‰๐š๐œ๐จ๐› ๐€๐›๐ž๐ฅ, who conceptualized dialysis, filtering waste from blood, a century before this life-sustaining treatment was used in patients with kidney failure in the 1950s.

The first description of dialysis, โ€œOn the Removal of Diffusible Substances From the Circulating Blood by Means of Dialysisโ€, in 1913 by Dr. John Abel and colleagues from Johns Hopkins University, was published in the Transactions of the Association of American Physicians.

Improved dialysis apparatus became practical for medical use once the materials cellophane (semi-permeable membrane), heparin (anticoagulant), and Teflon (non-stick cannula) replaced Abelโ€™s collodion membrane, hirudin (from leech saliva), and glass.

Immunosuppressant medications, while necessary for transplant recipients, can also lead to viral reactivation. Herpesviruses (e.g., EBV, CMV, VZV) establish lifelong latency and reactivate when immune control weakens in both spaceflight and immunosuppression. While reactivation is generally asymptomatic in astronauts, it is often symptomatic in transplant patients due to the higher, more sustained level of immunosuppression.
