From a compromised #44 to a stable and predictable restoration with AXEL.

Procedural Overview

✔ Prosthetic loading 2 months after surgery

Surgical Key Points

Lingual positioning of the implant created sufficient space for buccal GBR.

AXEL’s stress-free neck design provided additional space for predictable graft placement in the buccal area.

OVIS XENO-B was used to help maintain long-term volume and support the buccal contour.

Final Restoration

How Thread Design Transfers Force to Bone

In the second installment of the AXEL Implant Series, Prof. Moon-Young Kim examines how thread design determines the way force is transferred from implant to bone.

The article traces the evolution of thread shape from V-thread to Square, Buttress, and the modern Hybrid design, explaining how each geometry differs in shear versus compression, and why compression-dominant threads are the safer choice in compromised D3/D4 bone.

Drawing on a broad body of published research, the article also shows that thread pitch is not simply spacing — it determines how much bone is engaged and compressed between each thread — and extends this into the concept of variable pitch, combining a narrow coronal zone for stress control with a wide, deep apical zone for self-tapping and compression.

The key takeaway: thread shape, pitch, depth, and self-tapping geometry are not independent variables but must be optimized together as a single system, and thread geometry becomes the critical layer of control once surgical technique alone reaches its limits in extreme D4 bone.
