Not all PRP is created equal, and a new review makes that hard to ignore.
Researchers at Emory reviewed a decade of clinical evidence on PRP for facet-mediated low back pain (Chang et al., Biomedicines, September 2026). The outcomes were encouraging, but how PRP was prepared and delivered varied widely from study to study:
▪️ Platelet concentration: roughly 2× to 5× baseline
▪️ Leukocyte content: mostly leukocyte-poor, with no head-to-head comparison
▪️ Spin protocol: single or double spin, at forces from 200 g to 1,600 g
▪️ Injection volume: 0.5 to 2 mL per facet joint
▪️ Treatment protocol: one injection or a series, guided by fluoroscopy, ultrasound or CT
The authors name the lack of standardization in PRP preparation as “the most critical barrier to clinical translation.”
For clinicians, the takeaway is practical. The PRP system you choose shapes what goes into the syringe, and what goes into the syringe shapes outcomes. Your system is a clinical decision.
