Wound Slough: Why It Happens - and Why Moisture Balance Matters
In chronic and hard-to-heal wounds, one common challenge is slough.
Slough is soft, moist, non-viable tissue that may appear yellow, tan, gray, or stringy on the wound surface. It can form when dead tissue, inflammatory debris, bacteria, and excess wound fluid accumulate in the wound bed.
When slough remains in the wound, it can make assessment more difficult, interfere with healthy tissue growth, contribute to odor and drainage problems, and slow progress toward healing.
Non-viable tissue in a wound is not always just “dry dead tissue.” It can include a mixture of solid and viscous components - such as slough, necrotic tissue, and eschar - combined with wound fluid and inflammatory debris.
A convenient approach is to support the wound’s own continuous natural autolytic process.
Autolytic debridement uses the body’s own moisture and enzymes to soften and break down non-viable tissue gradually. For this process to work well, the wound environment should not be too dry - but it also should not be overwhelmed with excess exudate.
This is where dressing design matters.
ENLUXTRA® Self-Adaptive Wound Dressing is designed with Adaptive Moisture Control technology to help manage excess exudate while supporting a balanced moist wound environment.
Under ENLUXTRA®, the moist wound environment supports natural autolytic activity, allowing non-viable components such as slough and necrotic debris to soften and liquefy. At the same time, ENLUXTRA® is able to absorb and retain liquefied wound debris along with exudate, continuously removing all non-viable products of autolytic debridement away from the wound.
The goal is not simply to “dry the wound out.”
The goal is to help create conditions where non-viable tissue can separate more naturally, excess drainage can be managed, and the wound bed can become cleaner and stay clean over time.
For clinicians, this can offer a practical wound-bed preparation strategy between visits:
- support moisture balance
- manage exudate
- help reduce slough burden
- protect the surrounding skin.
Slough reduction is not just about removing tissue.
It is about creating a wound environment where healthy healing can move forward.

Maceration vs. Desiccation: Two Sides of the Same Wound-Care Problem

In wound care, moisture balance is everything.

Too much moisture can overwhelm the periwound skin, leading to maceration - fragile wound edges, leakage, odor, skin breakdown, and more frequent dressing changes.

But the opposite problem can also happen.

When clinicians try to prevent maceration by using highly absorbent conventional dressings, the wound may become too dry. This can lead to desiccation, where the wound bed loses the moisture needed to support natural healing.

That is one of the challenges with many conventional superabsorbent dressings: they are designed primarily to pull fluid away. While this can help manage drainage, excessive or non-selective absorption may also remove too much moisture from areas of the wound that need hydration.

And chronic wounds are rarely uniform.

One part of the wound may be heavily draining.
Another part may be dry, fragile, or trying to epithelialize.

That is why absorption alone is not always enough.

ENLUXTRA® Self-Adaptive Wound Dressing is designed with Adaptive Moisture Control technology to help manage excess exudate while supporting a balanced moist wound environment - helping protect against both maceration and desiccation.

For difficult, draining, or slow-healing wounds, the goal is not simply to absorb more.

The goal is to help maintain the right conditions for healing:
not too wet, not too dry - balanced.

ENLUXTRA®: advanced moisture balance for complex wound care.

Difficult wounds don’t always need more complexity.
Sometimes, the better option is a smarter dressing.
When managing heavily draining, chronic, or hard-to-heal wounds, don’t overlook ENLUXTRA® Self-Adaptive - wound dressings powered by Adaptive Moisture Control technology.
ENLUXTRA combines advanced moisture-balancing performance with the practical convenience of a wound dressing that fits under standard wound dressing coding groups, including A6196–A6197.
Why does this matter?
Because in home health, every unnecessary dressing change can mean more nursing time, more supply use, more scheduling pressure, and higher cost.
ENLUXTRA is designed to absorb and retain highest levels of exudate while helping maintain a balanced moist wound environment - not too wet, not too dry. Its Adaptive Moisture Control mechanism supports consistent wound conditions between visits and may help reduce dressing-change frequency when clinically appropriate.
For home health agencies, that can mean:
✔ Fewer unnecessary dressing changes✔ Less nurse workload✔ Better use of visit capacity✔ Lower wound care supply and labor burden✔ More predictable wound care workflows✔ Greater ability to manage difficult wounds efficiently
ENLUXTRA has been used on hundreds of thousands of challenging wounds across major hospitals and health systems - including Advocate Health, Adventist Health, Kaiser Permanente, Covenant Health, UnityPoint Health Avera, and many others.
It is also available through major distributors, including Medline, McKesson, and Cardinal Health.
Advanced wound care does not have to mean complicated workflows or premium-category pricing.
For difficult wounds, heavy drainage, and patients who need stable moisture balance between visits - remember ENLUXTRA.
Smarter moisture control. Simpler wound care. Better economics for home health.

07/05/2026

ENLUXTRA’s Adaptive Moisture Control was developed for one of the most important challenges in wound care: maintaining the right moisture balance.
A recent WoundSource article explains why wound exudate is not just “drainage” - it is part of the wound environment. Too little moisture can slow healing. Too much or poorly controlled exudate can contribute to maceration, tissue breakdown, delayed closure, and prolonged care.
That is exactly why ENLUXTRA is designed to do more than absorb. It helps manage excess drainage while supporting a stable moist wound environment between dressing changes - helping protect against both over-saturation and over-drying.
For clinicians managing chronic or heavily draining wounds, moisture balance is not a small detail. It is central to more predictable wound care.
Thank you to WoundSource for highlighting this important wound healing principle.
