March 2022 began with the end of the pandemic in sight, but we have finished the month with a large spike in cases as the highly transmissible Omicron variant and its sub-variant BA.2 spread through the UK and the rest of the world. The ‘Living with Covid’ strategy has been implemented by many countries across the globe, as the success of vaccination programmes continue as well as the dominant variant being less lethal. China has decided to continue with a zero-Covid strategy, which has found them insisting on mandatory tests as well as strict restrictions and lockdowns.

The spring vaccination programme has begun in the UK, with the over 75’s, adults in care homes and vulnerable people over the age of 12 being offered a vaccine, with plans to extend the roll out further later on this year. Scientists and doctors have stressed the importance of a further booster vaccination, due to the reduction in immunity of the vaccine over time, therefore reducing the chances of people becoming seriously ill and leading to a rise in pressure on the NHS.

In a continuation of China’s zero-Covid policy Shanghai is entering a lockdown this morning, making it the biggest city wide lockdown the country has seen since the start of the pandemic. The city has seen a significant hike in case numbers (although still relatively low by international standards) and will be locked down in two phases allowing for mass testing of its 25 million inhabitants.

With BA.2, a sub-variant of Omicron, causing the current surge in cases, Chinese authorities are concerned. However, with a policy that differs significantly from the rest of the world and a milder form of the virus, questions are being asked as to whether a zero-Covid policy is sustainable in the long run.

The World Health Organisation (WHO) has blamed rising Covid cases in Europe on some countries lifting restrictions too early, including the UK. There is now a sharp rise of infections across the continent, with countries feeling more pressure on their health systems and some care homes and hospitals closing off to visitors or closing down entirely due to increased numbers of staff contracting the virus. Health experts have said the rise in cases is also due to reduced immunity from the vaccine over time and the high transmission rate of Omicron and its BA2 sub-variant.

Here at Inspira, we have partnered with the best minds in virology, respiratory disease and inhaled drug therapies, with our shared mission to be part of the solution to a global problem. The IPX formulations that we have in-licensed for use in the field of respiratory disease are based on proprietary plant extracts that have previously been successful in the treatment of patients with skin disorders.

Our team has tested these IPX formulations against SARS-CoV-2 (Covid-19) and found the results extremely exciting, as we believe that our IPX formulations have the ability to destroy the viral spike proteins that facilitate infection. Moving forward, we are continuing to test and develop our technology in preparation for human trials.

As the rest of the world has opened up, China is continuing with its zero-Covid strategy. In recent weeks, China has been battling an outbreak of Omicron, the worst outbreak since the start of the pandemic. Millions of people have been put into lockdown and mass testing has begun. With limited people vaccinated (over half of over 80s, 15 million people, are unvaccinated), and a crumbling health care system, China is currently in a precarious position and the next few weeks will be vital.

New research has indicated that the global coronavirus death toll could be three times higher than official figures published. The study estimated that 18.2 million people may have died from Covid-19 by December 2021, opposed to the official recorded figure of 5.9 million deaths. The study looked at data from 191 territories between January 2020 and December 2021. The large differences between official data and the estimated figures could be due to under diagnosis from a lack of testing and problems reporting death data. Further research will be carried out as more data becomes available. Fully understanding the death toll of the pandemic is vital for public health decision making.

Researchers from Oxford University have found that Covid-19 can affect the brain. The researchers studied participants who had contracted the virus and those who hadn’t, and found that participants who had been infected by Covid-19 had brain shrinkage between 0.2 and 2%. The researchers also stated that these participants had lost grey matter in the areas that were linked to smell and memory, and even discovered that those who had only recently recovered from Covid, found it more difficult to perform complex mental tasks. It is important to remember that the researchers also pointed out that there is a good chance that the brain heals itself over time.

The pandemic has had a significant impact on the way society functions, from more people choosing to work from home to the increased use of technology for everyday tasks. The effects of the pandemic will certainly be long lasting, but there are likely to be a few positive effects as well.

At Inspira our main focus is conducting research and ensuring the ongoing development of our IPA formulation technology. Our plant-derived enzymes stem from papaya extract, which have proven to be an effective option in treating chronic skin conditions. We hope to apply this success to the treatment of respiratory disease, with our ultimate mission being to create an approved medicine for respiratory diseases such as influenza, cystic fibrosis and COVID-19.

A major milestone for us since the start of our journey has been conducting our in vitro tests at the University of Oxford. This research was designed to determine the virucidal activity of our formulations using live SARS-CoV-2 virus, the results were hugely exciting and provided us with confidence that our formulations will have the ability to rapidly degrade SARS-CoV-2 viral spike proteins. We've also seen early indications that our treatment works well against all variants, and we're hopeful it could become a pan-Covid treatment in the future.

February 2022 saw living with Covid become a reality, with many countries scaling back or even removing Covid-19 related restrictions.

The start of February saw the continued spread of the Omicron variant around the world causing many countries to continue with restrictions, resulting in increased frustration. Canadian truckers started a major protest against Covid-19 restrictions, influencnig other countries to do the same across the globe, from New Zealand to France. Yet in other parts of the world, such as Denmark, Covid-19 was no longer considered a ‘critical threat’.

In the UK trials of a new vaccine specific to the Omicron variant commenced, with members of the public volunteering. Studies such as these are necessary to test and monitor new vaccinations in order to improve our medical knowledge of the virus and to aid in preparation for any future epidemics. After research from the JCVI on vaccinating younger individuals, the UK approved the Covid-19 vaccine for children aged 5-11.

England has ended all legal restrictions in a move to ‘live with Covid’, no longer requiring individuals to isolate after a positive test. The removal of restrictions has been mirrored in other countries across Europe, such as Northern Ireland, Denmark and Germany. The UK has recognised that this tactic comes with risks, as the pandemic is still ongoing, and therefore has advised that vulnerable individuals get another booster vaccination in the Spring, followed by the rest of the population, in order to minimise any potential future risks.

As restrictions ease across the UK with the attitude of living with Covid becoming a reality; precautions still need to be made to ensure the pandemic remains under control. UK vaccine advisors have stated that a further booster jab is necessary to prevent severe illness from Covid-19. A spring booster jab will therefore be offered to the most vulnerable; the over 75s and individuals with underlying health conditions over 12. The Joint Committee on Vaccination and Immunisation (JCVI) have stated that it has made this decision as a precaution. Following vaccination of the most vulnerable, the rest of the population will be invited in stages to receive their booster, in order to better protect the population as well as best avoid the possibility of further mutations.

Covid-19 has been the dominating concern for healthcare systems around the world for the past two years. This has meant that waiting lists have increased and those in need of treatments for other conditions are having to wait longer to be seen and treated.

The pandemic has not only caused a major health incident in the short term, it will have a long lasting effect for years to come.
