Most AI agents don’t fail because the technology is wrong.
They fail because nobody connected them to anything real.

A customer asks why their bill went up.
The agent checks usage, cross-references the tariff, responds in seconds.

No ticket. No queue. No human pulled away from something else.

That is not a future state.
That is what happens when the agent is actually wired to live data.

One of Britain’s largest energy providers was falling apart under its own demand.
Millions of households. One platform. Breaking at the seams.

We rebuilt it from the ground up. Not a patch. A new foundation.
The architecture their product teams still build on today.

Most businesses are not behind on AI.
They are running agents that only worked in the demo.

Autonomy is earned. Not switched on.

Fable 5 just launched. 90% of businesses will not see a difference.
Not because the benchmarks are wrong, but rather it’s something else.

A more capable model does not fix a broken process.
It moves faster through the same gaps.

No decision layer means the output lands and nothing happens.

Messy data in still means messy output out.
The model finds the problem quicker.
The problem is still there.

The businesses that will benefit had three things in place before the upgrade dropped.

Clean inputs. Clear output handling. A decision layer that acted on results.
The model just slotted into a system that was ready for it.

Most teams are asking which model to use.
The real question is whether your system can use what you already have.

The winners were not waiting for a better model.
They built the system first.

Microsoft basically just stopped outsourcing its brain.

For years every Microsoft AI product ran on OpenAI.

OpenAI controlled the price. OpenAI controlled the direction. Microsoft just had to go along with it.

Then on 2 June they launched two of their own AI models.

One writes code. One handles complex thinking tasks. Both cheaper than what they were using before.

Think of it like ordering takeaway every night from the same place then one day learning to cook. Same meal. Way cheaper. You’re in control.

Here’s what that means in practice.
Everyday AI tasks get around 30% cheaper. You rely less on one company making decisions for you.

You stop paying premium prices for tools you only need some of the time. More options, more control, less drama.

And this isn’t just about saving money.

Microsoft went from reselling AI to building it.

For businesses already using agentic AI or workflow automation, this changes which tools sit inside your stack and what you pay for them.

If your business runs on Microsoft 365, the way you buy AI tools is about to change. The best deals are on the table right now. They won’t be for long.

Are you still running your business on one AI tool? Most people are.
Most people don’t know the risk.

The people building AI are telling us to slow down.

Not because the technology is failing.

Because nobody knows how to control it yet.

Jack Clark, co-founder of Anthropic, said this out loud on BBC Newsnight last week.

A person who has spent years building some of the most powerful AI in the world, saying the industry has no brake pedal.

And here is what that gap actually looks like.

Every few months, AI gets meaningfully more capable. The rules and checks that are supposed to sit around it move way slower.

So the gap between what AI can do and what anyone can actually control just keeps getting wider.

And honestly, nobody is being reckless on purpose.

Building the capability is faster and more exciting than building the guardrails.

That is just human nature. We build the thing first and figure out the safety net later.

The problem is that pattern does not stay in Silicon Valley.

It shows up inside businesses too.

A tool gets deployed. People start using it. Life gets busier. And nobody ever stops to ask who is actually checking what this thing produces.

No record of what it decided. No review. No clear line between where the AI stopped and where the human took over.

Most of the time it is fine. Until one day it is not, and by then it is hard to even trace what went wrong.

The fix is not a big project. You just need to know what your AI handles alone, what needs a person involved, and keep a record so nothing disappears.

Then check it before something breaks, not after.

That is what a brake pedal looks like.

Jack Clark is asking governments to build one for the whole industry.

That might take years.
Yours does not have to.

Somewhere in every growing business there is a task that keeps landing on the wrong person.

Not because the team is not capable. Not because the tools are not there. But because nobody has built the connection between them.

That is the gap. And most businesses have stopped noticing it.
We saw it play out on a global trading floor.

Tens of millions of trades processed every single day. A compliance team pulling data by hand.

The anomalies were sitting in the system the entire time. Nobody was looking in the right place.

The risk was not a missing tool. The AI existed. The data existed. What was missing was the layer that connected them and made them work together.

So we built it. One agentic pipeline watching every trade, every venue, every pattern in real time. No manual pulls. No spreadsheets. Numbers the team could finally trust.

Compliance workload dropped by over 70%.

Anomalies caught before they escalated. Manual reporting eliminated completely.

The team stopped firefighting and started doing the work they were actually hired to do.

That is what closing the gap looks like in practice.

At BraivIQ we build the connection layer between the AI your business already has and the work that still needs doing.

Production-ready AI systems that run where the work actually happens.

The gap is closable. We have done it before. We will do it again.

Your team has six AI tools and everything still goes through you.
That is not a training problem. It is a structure problem.

Here is the simplest way to see it: imagine hiring someone brilliant. Smart. Capable. Ready to work.

But every decision has to go back to you first. After three months they are just filling a seat.

That is exactly what happens to AI inside a business with no clear decision structure.

Every workflow change needs approval.
Every question sits in a meeting queue. The tool waits.
Not because it does not work.

The businesses getting real results from AI are not running better tools. They mapped where decisions were stalling.

They decided what could move without sign-off. Then they built systems around how the team actually works.

Most AI projects don’t fail. They just never get started.

80% of agentic AI programmes fail within 90 days.

Not because the agents were bad. Because nobody built what sits underneath them.

Most teams spot a use case. Pick a vendor. Ship it. It works in the demo. Then it hits production. And it falls apart.

So here is what is actually going on.

Agentic AI is not one thing. It is three layers. Each one holds the next.

Layer one is Orchestration. It reads the goal, breaks it into steps, and routes each agent in the right order. Skip this and agents act out of order. Or act when they should not.

Layer two is Action. This is where agents touch your real systems. Your CRM. Your inbox. Your data. There is a standard called MCP that controls this. It defines what each agent can touch. And logs every action with a reason.

Layer three is Governance. Named owners. Cost limits. Kill-switches anyone can trigger, no ticket needed. Most teams write this last. It needs to be written first.

At BraivIQ, we build all three layers in order. Foundation first. Agents on top.

Which layer does your team skip first?

50 NHS organisations are already using AI to decide how urgent your case is. Most patients have no idea.

It is not every trust. Not yet. But it is already running quietly across England.

AI tools are reading chest X-rays across 40 trusts, flagging which surgery patients are highest risk, and cutting GP waiting times from 11 days to 3.

One GP practice in Surrey did exactly that.

Think of it like a second doctor who never sleeps and never misses a pattern. Sounds good, right? Here is the bit that should make you think.

It cannot explain its own reasoning. It flags your scan. It cannot tell you why.

That gap between what AI decides and what it can explain is the conversation most people are not having yet.

The NHS is not trialling AI anymore. It is already in the room with you.

You just were not introduced.

Most businesses think the AI model is the hard part. It is not. Claude Opus 4.8 is built to run hundreds of tasks at the same time.

Legal review, financial checks, compliance, content. All at once, not one after the other.

But most businesses are still running everything in a queue.

One task finishes. The next one starts. That is the slowest possible way to use AI.

Think of A&E. You do not wait for one doctor to finish every patient before they see you.

A triage nurse routes each person to the right room immediately. Multiple doctors work in parallel.

That routing decision is the hard part. Not the doctors.

Businesses applying this approach have cut document reviews from weeks to under two hours.

Not by getting a better model, but by building the right structure around it.

Most people are optimising the model. The ones getting results are fixing the queue.

The model was never the problem. The queue was.

You are paying for AI tools. And that has not changed how a single hour gets spent in your workflow.

Your team opens the tool. Closes it. Does the work the old way. That is not a people problem. That is a structure problem.

The structure has three gaps.

The first is visibility. You map every manual task and put a time cost next to it. That tells you exactly where AI goes first.

The second is capability. Not a course. Your team works inside the tools on real production from week one. The habit only forms when the stakes are real.

The third is automation embedded into the operation, tracked every week. If nothing is measurable in seven days, the deployment is wrong.

The businesses seeing results did not buy better tools.

They fixed what the tools were dropped into.

Which of the three gaps is your business stuck at right now?
